PLC Application - Complete this form if you are currently a freshman, sophomore, or junior in college, or are a 1L or 2L law student. Consult the Application Instructions before you begin.
OCC Application - Complete this form if you are a college or law school graduate, currently not enrolled in school. Consult the Application Instructions before you begin.
PIQ's - If you are PLC or OCC, you will need to complete at least five of these forms. Consult your Application Instructions for details.
Pre-Ship Checklist - This is the form you will fill out after your pre-ship PFT, approximately one month before shipping to OCS.
Request for Appointment - Fill this out AFTER you are fully trained, to set a date for your commissioning. Do NOT use this form to set an appointment with the office--simply Contact Us.
